With a member of Heritage Medical Group as your primary care physician, your health and that of your family is on the right path – particularly when you schedule regular check-ups and follow guidelines he or she establishes.
If it is determined that you require hospitalization, your primary care physician will coordinate your care or refer you to a Heritage specialist. This will help ensure that the level of care you have come to know as a patient of Heritage Medical Group will continue during your hospital stay.
With privileges at both PinnacleHealth Systems and Holy Spirit Health Systems, Heritage providers can select the hospital setting that will best suit your needs, at your option.
Even if your primary care physician does not see patients in the hospital, you will be seen by one of Heritage Medical Group’s physicians.
This is also important to know if you go to one of the hospitals’ emergency rooms and need to be admitted for adult medical care, cardiology or general surgery, or admission of your child for pediatric care. Simply ask that medical care be carried out by a Heritage Medical Group physician, and the emergency room physician will make those arrangements.
After your discharge from the hospital, your primary care physician can coordinate any necessary follow-up testing, office appointments or ongoing outpatient care that has been prescribed.
The Heritage physician who cared for you in the hospital will share all pertinent information with your primary care physician. This communication will help ensure a seamless post-hospital transition – another benefit of selecting Heritage physicians for your medical care.
